Monitoring Performa

I am trying to design a Performa for monitoring purpose. I am unable to really design one. Please anyone help me out in this. The Performa is to check progress of different development projects from an NGO viewpoint. How can this be designed? What points to be put? And how can it be replicated somewhere else?

IF anyone has a prepared Performa, please share it with me.
